Abstract: Hypocrites is noted for famously stating that healthcare should 'not harm'. Today, sadly the opposite appears to be true. Especially in the US, progressively the healthcare system, due to lowering quality of services, inadequate access to services, and escalating costs of treatments, at least on face value is doing great harm. Lack of respect for the patient appears to be the key problem. To address this, we proffer a new model of care; namely care coordination enabled through digital health to realise the attainment of high-value, patient-centred care for anyone, anytime, anywhere. Our model was developed by systematically analysing literature and then synthesising it with a focus on current problems and barriers in existing models of care. Thus, the following presents our proffered model, an encircled care contribution measure share model. This model enables high-value patient-centred care to ensue through its ability to identify objective tracking of clinical services, examine by stakeholder input/feedback and assess these concerning key metrics measures; from which both value and quality can be identified.
